| Anotation | The main objective of the project is to create and verify the effectiveness of a community group program focused on supporting self-management in older adults with essential hypertension to improve their health behavior, health literacy, adherence, self-care, self-efficacy, health responsibilities, mental health, clinical parameters, and quality of life. Another objective is to identify factors affecting the level of self-management of chronic disease and barriers to adherence to preventive measures. An observational cross-sectional study, a qualitative study and, subsequently, a randomized controlled intervention study will be conducted. The preventive intervention program will be designed based on age-specific recommendations and identified knowledge gaps of older adults with essential hypertension. The program will run for at least 3 months. The parameters to be studied will be evaluated before the intervention, after the intervention, and at three and six months after the intervention. Quantitative analysis will be used to assess the effectiveness of the intervention provided on changes in monitored variables will be assessed. Qualitative methods will be used to evaluate patient satisfaction with the program developed and barriers to implementation of secondary prevention recommendations. The sample will include older adults (?60 years) diagnosed with essential hypertension who will be addressed through the Center for Prevention of Chronic Disease and Promotion of Healthy Aging at the Faculty of Medicine, University of Ostrava. Based on the results, recommendations will be made for a group program supporting self-management of chronic disease as part of secondary prevention in older adults with hypertension. The project builds on an intervention study carried out at the Centre for the Prevention and Support of Healthy Aging aimed at evaluating the effect of selected psychosocial interventions on the mental health of older adults. |
