| Anotation | High turnover and burnout among healthcare workers (HCWs) threaten the stability of the healthcare system, workforce sustainability, and care quality. The intent-to-leave the profession is shaped by individual, organizational, and systemic factors, which have not been sufficiently explored in Czechia. Objective: To identify key individual, work-related, and structural factors that shape the career paths of physicians and nurses, from the transition to clinical practice to long-term workforce retention. Methods: We will conduct a longitudinal follow-up of the HEROES-CZ cohort (n=3,624) to assess changes in intent-toleave and related risk factors (burnout, stress, workplace support). A new cross-sectional study (n=1,000) will compare trends in intent-to-leave with data from HEROES-CZ. Using semi-structured interviews with HCWs who have left the profession (n=10-15) we will analyze their analyze their decision-making. A new longitudinal cohort of medical and nursing students (n=1,000) will be tracked at three points: at the end of their studies and twice during their first year of practice, to assess transitions to clinical practice and the risks of early career attrition. Case studies of hospitals with low turnover will reveal effective strategies for workforce stabilization. Quantitative analyses will use mixed-effects models and logistic regression, while qualitative data will undergo thematic and content analysis. Relevance: The project combines quantitative and qualitative approaches to identify effective strategies for retaining HCWs. In addition to 5 scientific articles, practical outputs will include workshops for medical students and nurses (n=6), a toolkit for educational institutions, and a report on best practices for stabilizing the workforce. The findings will aid policymakers, healthcare institutions, and educational bodies in developing sustainable strategies for retaining HCWs and strengthening the Czech healthcare workforce. |
